Collecting Feedback with Intake Forms

Idea Manager lets you easily create custom, web-based intake forms to collect feedback. Each intake form is associated with specific product areas, so you can:
Create custom intake forms based on a specific project, feature, etc.
Create as many intake forms as you need for a product area
Customize each intake form with specific feedback requests, fields, and more

Creating an Intake Form
To create a new intake form, go to the Portals page and select + Add Intake Form.

Intake Form Details
Enter details for the intake form:
Name - This will be displayed at the top of the published intake form.
Intake Form URL - The URL is automatically created based on the form’s name, but you can edit it as necessary. The base URL cannot be changed.
Product Areas - Assign the intake form to product areas. This will connect any feedback you receive to those product areas.

Only people who have access to a selected product area will be able to see feedback for that product area. To adjust a product area’s permissions, see Product Areas.
Intake Form Content
You can customize the intake form to ensure you’re receiving the feedback you need:
Add a form image - This will appear at the top of the intake form.
Add form description—This is displayed below the name and can provide additional context for your customers. Note that this is different from the Description field, which is where respondents will enter their feedback.
Add field - You can select from existing fields, or create custom fields just for this form.
Allow submitters to upload files to their response - Toggle this option to enable/disable file uploads with feedback.

Once you’re finished, select Publish.
You cannot edit the Summary or Description fields. These fields are where the form recipients will enter their primary feedback.
Publishing an Intake Form
Once you’ve finished customizing your intake form and clicked Publish, you can copy the form link and use it to solicit feedback.

To test your new form, click See Published Form.
